Total testosterone
All the testosterone in your blood: the part carried by proteins and the small part that circulates free. Levels run highest in the morning, which is why tests are usually drawn early.
The clock changes the answer
In men aged 30 to 40, total testosterone measured at 4pm runs 20 to 25 percent lower than the same man at 8am. In that study, 17 men with an afternoon reading below 300 ng/dL were normal at all three of their morning draws.

Same blood. Different answers.
One man's blood sample was sent to 1,133 labs using 14 different test methods. The answers came back anywhere from 45 to 365 ng/dL. The highest was eight times the lowest.

A single draw is not an answer
Roughly 30 percent of men whose first testosterone lands in the hypogonadal range come back normal on a second morning measurement. This is why the guideline asks for two separate fasting morning draws before anything else.
